About Your Personal Health Assessment (PHA)

The Personal Health Assessment (PHA) is a complete health screening tool that identifies risk factors such as high blood pressure, high cholesterol, diabetes, obesity and much more.

Awareness is the first step toward a healthier lifestyle and lasting change. Begin your journey towards optimal health with a Personal Health Assessment. 

After completing your PHA, you will receive a text message indicating your results are available. Log in to the CareATC mobile app or the CareATC Patient Portal to access your results. If you opted for a paper booklet, it will arrive at your home by U.S. mail within two weeks.

Rewards & Incentives

Employees and covered spouses who complete the PHA by the annual deadline will pay significantly less for medical coverage.

Schedule an appointment at one of our national CareATC Locations. Visit the Patient Portal online or schedule an appointment on the CareATC mobile app. 

3) Labcorp Locations

This option is only available for select employees. If you are eligible, select to complete your PHA at 'Another Location' and select the LabCorp location option that works best for you. For more detailed instructions, click here.
