How to Register for Your 2021 Wellness Screening (Personal Health Assessment / PHA)

Completing the wellness screening is the first step toward earning your $700 health incentive. There are three ways for you and your spouse to participate in your wellness screening:

Come to an on-site wellness screening at a location near you (more information to come), schedule an appointment at one of the NextEra Health & Well-Being on-site health clinics for a lab work screening, or schedule an appointment at a designated LabCorp using the CareATC app or online Patient Portal.

After completing your wellness screening, schedule your annual physical with your primary care provider or at one of the NextEra Health & Well-Being on-site health clinics.

Take the following actions to access a designated LabCorp location to complete your wellness screening (PHA / lab work):

1. Go to www.careatc.com/patients

2. Sign in using your username and password or register to create an account.

3. Once signed in, select the tile "Schedule your Wellness Screening“.

4. Next, select the option for "At Another Location".

5. Then, select "LabCorp" as the outside facility for your wellness screening.

6. Next, verify your information and complete the authorization.

7. Verify or enter your zip code and hit continue and you will then be routed to LabCorp's website.

8. Select the "New Search" option.

9. Add in your zip code or click locate me.

10. Select "Employee wellness with body measurement".  Choose the radius for miles and then select search.

11. Make appointment at the desired location by clicking the blue "Make Appointment" option.

12. Select "yes" to fasting and choose a date and time.

13. Enter the requested patient information.

14. Select "I have already paid or someone else is responsible" under financial details.

15. Review your reservation and select Create Appointment.

16. LabCorp will receive the information for what lab work to order in connection with your wellness screening appointment.

17. You will then be brought to a confirmation screen with your appointment details that you can bring with you on the day of your scheduled appointment.

18. You will also receive an email confirmation from LabCorp that you can bring with you on the day of your scheduled appointment.
