Scheduling an Appointment
There are 3 easy ways to schedule an appointment:
1) Call the CareATC Patient Access Center at 800.993.8244;
2) Visit the Patient Portal and use the unique code issued to you on your PHA report; or
3) Download the CareATC Mobile App - after signing in, use the app to schedule an appointment anytime, anywhere.
About the CareATC Mobile App
You can download the CareATC mobile app on the Apple App Store or Google Play Store.
Accessing Your Personal Health Assessment
A highly confidential summary is provided to each participant, displaying screening results and tips for improvement. Your PHA Summary Booklet will arrive in approximately one week after your draw. Your booklet will list your results and show you where they fall on the scale. You can also access your PHA results on the CareATC Mobile App.
About Prescription Refills
Patients can refill CareATC prescriptions in two to three days by calling the Patient Access Center, or by request through the convenient mobile app. Often, no Wellness Center visit is necessary unless otherwise required by the physician.
